MetaWell PLLC – Terms of Service

1. Overview
Welcome to MetaWell PLLC (“MetaWell,” “we,” “us,” or “our”).
MetaWell provides physician-led telehealth medical services focused on men’s health, including hormone optimization, metabolic health, and sexual health.
By accessing this website, scheduling a consultation, or using our services, you agree to these Terms of Service.
2. Medical Services Disclaimer
MetaWell provides medical consultations and care by a licensed physician.
Purchasing a consultation or enrolling in services does not guarantee:
- a diagnosis
- treatment
- prescription medications
All medical decisions are made based on clinical judgment, medical appropriateness, and patient-specific factors.
MetaWell does not provide emergency care.
If you are experiencing a medical emergency, call 911 or seek immediate care.
3. Eligibility
To use MetaWell services, you must:
- Be at least 18 years of age
- Provide accurate, complete, and current information
- Be physically located in a state where the provider is licensed at the time services are rendered
MetaWell reserves the right to refuse service if these conditions are not met.
4. Telehealth Services
By using MetaWell, you consent to receive care via telehealth, which may include:
- video visits
- phone consultations
- secure messaging and electronic communication
You acknowledge that:
- telehealth has limitations compared to in-person care
- certain conditions may require in-person evaluation
- care may be modified or declined based on these limitations
5. Payment Terms
- A $99 consultation fee is required prior to scheduling
- Payment is processed through a secure third-party processor (e.g., Stripe)
- The consultation fee may be applied toward your first month of care if treatment is initiated
Additional services, memberships, or treatment plans may involve recurring or additional charges, which will be disclosed prior to enrollment.
6. Membership and Subscription Services
Certain services may be offered through a recurring membership model.
By enrolling in a membership:
- you authorize recurring billing
- you understand that services are provided on a subscription basis
- you agree to the terms outlined in the Membership Agreement
Membership fees are not contingent upon receiving a prescription.
7. Refund Policy
- Consultation fees are non-refundable once the consultation has occurred
- Membership fees are non-refundable once services have been rendered or medications dispensed
- No prorated refunds are provided for partial billing periods
Refunds, if any, are issued at the sole discretion of MetaWell in limited circumstances.
8. No Guarantee of Treatment or Results
MetaWell does not guarantee that:
- you will qualify for treatment
- you will receive a prescription
- any specific outcomes or results will occur
Treatment recommendations are individualized and based on medical evaluation.
9. Prescriptions and Pharmacy Services
If treatment is prescribed:
- prescriptions are issued only when clinically appropriate
- medications are fulfilled by licensed third-party pharmacies
- MetaWell does not manufacture or directly sell medications
Medication availability, pricing, and shipping timelines are controlled by third parties and may vary.
10. Controlled Substances
Certain medications prescribed through MetaWell may be classified as controlled substances.
By receiving such prescriptions, you agree to comply with all applicable policies, including the Controlled Substance Agreement.
Failure to comply may result in discontinuation of care.
11. Patient Responsibilities
You agree to:
- provide accurate and complete medical information
- follow medical advice and treatment plans
- complete required laboratory testing
- use medications only as prescribed
- communicate any adverse effects or concerns
Failure to comply may result in modification or termination of care.
12. Communication
MetaWell may communicate with you through secure messaging, phone, SMS, or email.
You agree to:
- use communication tools appropriately (non-emergency use only)
- understand that responses are not immediate
- comply with the Communication Consent
13. Privacy
Your medical information is handled in accordance with applicable privacy laws and our Privacy Policy.
While MetaWell uses secure, HIPAA-compliant systems, you acknowledge that electronic communication carries inherent risks.
14. Third-Party Services
MetaWell relies on third-party providers, including:
- laboratories
- pharmacies
- payment processors
- communication platforms
MetaWell is not responsible for delays, errors, or issues arising from these third-party services.
15. Limitation of Liability
To the fullest extent permitted by law, MetaWell shall not be liable for:
- indirect, incidental, or consequential damages
- delays or issues caused by third-party providers
- outcomes resulting from failure to follow medical advice
- technical failures related to telehealth services
16. Dispute Resolution
Any disputes arising from these Terms or services provided by MetaWell shall be resolved through binding arbitration in the State of Tennessee, unless otherwise required by law.
By using our services, you waive the right to participate in class-action lawsuits.
17. Modifications
MetaWell reserves the right to update these Terms at any time.
Continued use of the website or services constitutes acceptance of any updated Terms.
18. Governing Law
These Terms are governed by the laws of the State of Tennessee.
